Red flags
  • Not weight bearing
  • Acute severe unilateral hip pain
  • Hot/painful/swollen joint with little movement
  • Bony tenderness/swelling
  • Nocturnal pain
  • Severe progressive symptoms
  • Back pain and/or oss of bladder/bowel control
  • Systemic symptoms
    • (fever, night sweats, weight loss)
  • History of Osteoporosis , TB, HIV, Cancer
Investigations to consider
  • FBC, CRP/ESR
  • U&Es, LFTs, Bone profile
  • Auto-antibody screen, Rheumatoid factor +/- anti-CCP antibodies
  • X-ray (hip & pelvis)
  • MRI scan
  • Bone scan

  • Many diagnoses are not obvious or apparent at the initial presentation. It may be appropriate to share your diagnostic uncertainty with patient, and also provide specific guidance (safety netting) on when and where patients should seek further advice and follow up.
