  • Painless progressive jaundice
  • Abdominal/liver mass
  • Weight loss
  • Systemic features (fever, night sweats, anorexia)
  • History of Drug & Alcohol misuse
  • Children
    • First 24 hours after birth
    • Prolonged jaundice >2 weeks
    • Poor feeding/weight gain
    • Lethargy
Investigations to consider
  • FBC, CRP/ESR, blood film
  • Reticulocyte count
  • Clotting screen
  • U&Es, LFTs, GGT, Amylase
  • Auto-antibody screen
  • Serum iron and TIBC
  • Serum ceruloplasmin
  • Serum a1-antitrypsin
  • Viral serology (Hepatitis screen)
  • USS
  • ERCP
  • MRCP
  • CT scan
  • Liver biopsy

  • Many diagnoses are not obvious or apparent at the initial presentation. It may be appropriate to share your diagnostic uncertainty with patient, and also provide specific guidance (safety netting) on when and where patients should seek further advice and follow up.
